2018 Clinician's Guide to PAP Adherence

The AARC and its members have long recognized that patient adherence for management of sleep-disordered breathing is at an unacceptable level—about 50% discontinue positive airway pressure therapy within the first year. The AARC has developed the following guide to train clinicians how to communicate with sleep patients, identify reasons why adherence is poor, and intercede with interventions that may improve compliance.

  • Cite the risk factors commonly associated with sleep-disordered breathing (SDB)
  • Identify the cardiovascular comorbidities frequently associated with SDB in adults
  • Explain the mechanism by which SDB is thought to lead to or aggravate Type 2 diabetes
  • Given a comorbidity commonly associated with SDB, explain how treatment for SDB may result in improvement of the comorbid condition
  • Recognize the common history and physical findings that lead to strong suspicion of an SDB diagnosis
  • Cite the criteria that must be met to establish a definitive diagnosis for SDB

  • Describe the basic differences between types of positive airway pressure (PAP) devices used to treat SDB

  • Identify common problems reported by patients receiving PAP therapy and recognize possible causes and solutions to these problems
  • Identify needs to be considered when offering interface options to the patient to maximize comfortable and proper fit
  • Describe the most effective method of introducing a patient to PAP
  • Describe the "Ask-Provide-Ask" patient interaction technique
  • Describe the four basic skills of motivational interviewing
